Home Health RN:
Coordinates the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing and evaluating. Provides leadership for the Health Care team by working cooperatively with other team members in maintaining standards of professional nursing practice in the Home Care setting.
Minimum or Preferred Qualifications Associates Degree, plus 1-5 years of relevant experience, Current license required

Home Health LPN
Under the supervision of a Registered Nurse, coordinates the Plan of Care for the patient in the home, hospital or other setting assessing, planning, and implementing. Works cooperatively with other team members in maintaining standards of professional nursing practice and in accordance with age, specific needs and within the scope of practice for License Practical Nurse within New York State.
Minimum or Preferred Qualifications Minimum one (1) year experience in Acute Medical/Surgical Nursing. Experience for private duty LPN will be evaluated.

Home Health Aide – Starting at $13.50 Full time/ Per diem
To provide personal care, health-related tasks in accordance with age-specific needs, and housekeeping services to clients in their homes, under the supervision and direction of a licensed professional nurse or appropriate agency professional therapist.
Minimum or Preferred Qualifications High School diploma. Previous work with elderly or chronically ill persons preferred. Certificate of completion of approved NYSDOH Home Health Aide Training Program. Ability to read, write and follow directions. Maturity, emotional and mental stability, sympathetic attitude toward caring for clients in their homes.

Hospital Liaison LPN
The Hospital Liaison Nurse LPN coordinates and facilitates the delivery of homecare services in conjunction with the homecare team, hospital discharge planning staff and other referral sources. The Hospital Liaison LPN facilitates referrals, coordinates skilled and non-skilled services, and other community resources through collection and input of data, interview and team collaboration in a variety of settings. The Hospital Liaison Nurse LPN performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse.
