Salary Range: $20.50 - $23.15 per hour
Family Choice of New York is looking for a Care Management Coordinator .The Care Management Coordinator is primarily an office-based role but may include off-site visits.
Care Coordination activities may be required for various programs and the program specific details of the role may vary from program to program.
The primary duties of the Care Management Coordinator are to:
• Coordinate the care delivered by others to an assigned caseload of members.
• Gather information necessary for effective and accurate care coordination.
• Routinely interface with member’s physician/physician offices and other service providers to assist with the coordination of care.
TASKS:
• Conduct a Health Risk Assessment Tool (HRAT) per Family Choice guidelines and ensure documentation and signature in the Family Choice care management record.
• Ensure individualized care plan is updated and maintained per Family Choice guidelines and signed by RN or licensed member of the IDT.
• Ensure that updates to the ICP are shared with members of the ICT including PCP per Family Choice guidelines and as stated in Model of Care.
• Outreach per Family Choice guidelines when a beneficiary is hospitalized.
• Complete all required documentation in a timely manner per company guidelines.
• Maintain an accurate record of telephonic interactions with members, the member’s PCP or other specialist providers and family/caregiver.
• Establish trust and rapport with members /families (as appropriate) and staff in each care setting in which members receive services.
• Identify whether the member is receiving care services as ordered.
• Coordinate and facilitate members’ access to services with other health care providers as needed. Ensure the proper HIPAA documentation has been completed as per company guidelines.
• May assist with coordinating hospital diversions when requested by the management team.
• Identify and address social determinants of health needs for members and refer to Social Work as appropriate.
• Identify potential clinical concerns and escalate to the clinical team promptly so that further direction can be given.
• Assist with scheduling in-home visits for other care management staff.
• Assist with arranging DME, transportation, and other services as requested by the Interdisciplinary Care Team.
• Maintain databases on members and complete accurate and timely documentation.
• Participate in Interdisciplinary Care Team (ICT) Conferences.
• Assist with IHA initiatives as requested by Management Team.
• Assist other team members as needed.
• Attend meetings as requested.
• Assist with training and oversight of new team members per management’s request.
• Provide back-up assistance for all FCNY programs.
• Other duties as assigned.
EDUCATION: Associate’s or Bachelor’s degree preferred.
EXPERIENCE/ABILITIES:
· Minimum two (2) years working in a call center and/or an administrative support role preferably in a health care setting.
· Excellent verbal and written communication skills.
· Ability to articulate and deliver accurate and clear communications via written and/or oral communication.
· Demonstrated pleasant telephone manner.
· Versatile and well organized.
· Requires strong problem-solving skills.
· Demonstrate talent for planning, initializing, and implementing operational processes and procedures.
· Strong time management skills.
· Must be able to build and maintain collaborative, cooperative, and productive relationships with all internal and external customers including staff, members and their families, nursing facility staff, physicians, the health plan, and any other stake holders in the community.
· Strong keying/data entry skills necessary.
· Computer literate with proficiency in a wide variety of applications.
· Must be flexible and able to work in a team environment.
REQUIREMENTS:
· CPR certification
· Valid NYS Driver’s License and active automobile insurance to meet company requirements
· Transportation
Job Type: Full-time, 40 hours per week
Pay: $20.50 - $23.15 per hour
