Job Description:

The full & part time Hospice Facility Registered Nurse accepts responsibility for the delivery of patient care through the process of assessment, evaluation planning, and implementation. Shift is 2 days a week: 6am- 7pm (days to be determined) and every third weekend. This RN is responsible for the management of the hospice facility staff in the absence of the Hospice Facility Care Coordinator.

Duties and Responsibilities include, but are not limited to:
• Plans and/or monitors delivery of skilled services to ensure positive patient outcomes and directs staff towards completing the same.
• Monitors medical condition of patients and ensures that hospice staff report appropriate changes.
• Assists with or directs admission/discharge of patients.
• Coordinates with nursing staff to ensure nursing and aide care plans reflect hospice needs and monitors the delivery of care to hospice facility patients and families.
• Ensures an adequate stock of supplies and proper functioning equipment.
• Assists physicians and NP with examinations, procedures, and processes related to direct hospice patient care.
• Communicates patient needs to other members of the interdisciplinary care team (i.e., Social Work, and Spiritual Care) and reports appropriate concerns to Coordinator of Hospice Facility Services.
• Intercedes with families and co-workers as situations require with goal of immediately resolving the same.
• Provides oversight and direction to hospice staff and provides teaching and mentoring.
• Maintains a safe, comfortable and therapeutic environment for patient/families in accordance with facility standards.
• Assist with the orientation, instruction and training of assigned personnel.
• Responsible for securing Hospice facility staff for the upcoming shift in the event of calls- ins or emergencies.

Education Requirements: High School Education or equivalent. Graduate of an accredited school of professional nursing

Work Experience: Five (5) years current clinical experience, preferably two (2) years in an acute care setting or skilled nursing facility, one year managerial responsibility in either an acute or skilled nursing facility

Licensure/Certification: RN Licensure in New York State (required) and Basic Life Support (preferred)
