Family Choice of New York is looking for a Advanced Practice Provider Clinical LeadThe primary role of the C-SNP Advanced Practice Provider (APP) Clinical Lead is to conduct on-site assessments and/or desktop case reviews of members who are followed in the care management program. The C-SNP APP Clinical Lead will provide guidance and support to the CSNP Care Management team. In addition, the CSNP APP Clinical Lead will manage members who are clinically complex and assist the clinical team with these members. This position is also responsible for ongoing focus on the achievement of desired clinical outcomes.
This position is a working lead role with time split between clinical accountability and lead responsibilities.
Nurse Practitioner/Physician Assistant Responsibilities:
• Conduct physical assessments on members as assigned by level of care.
• Communicate identified medical concerns to the member’s PCP and write orders as requested.
• Communicate regularly with the enrollee’s PCP based on the PCP’s preferences as documented in the Physician Agreement and as specified in the Model of Care. This includes documentation in the enrollee’s electronic record per company guidelines.
• Complete field visits for level 4 C-SNP members and/or as requested based on acuity and assist with discharge planning.
• Complete a root cause analysis and advise level changes as indicated.
• Discuss and document any problems or concerns identified by the member and/or other members of the ICT.
• Identify changes in health and report to the appropriate medical personnel.
• Complete all required visits, forms and documentation in a timely manner per guidelines and company policy in the appropriate electronic medical record.
• Identify and report changes in health to the appropriate medical personnel.
• Contact and maintain communication with member, family, and/or caregiver.
• May conduct Health Risk Assessment (HRA) with members per company guidelines and policy.
• Development and ongoing modification of the individualized care plan (ICP).
• Make clinical recommendations in accordance with policy, procedure, and within the scope of practice.
• Serve as a clinical resource for nurses and care coordinators.
• Perform in home acute visits to support diversions from home to sub-acute setting.
• Collaborate with clinical leadership and IHA UM Department to ensure diversion criteria is met and approved prior to completing process.
• Provide guidance to team members in the Transitional Care Management of members.
• Attend required meetings as identified by program management.
• Develop and maintain a good working relationship with community organizations.
• Complete goals of care discussions as identified in IDT or at the request of clinical leadership.
• Other duties as assigned.
Clinical Lead Responsibilities:
• Mentoring nurses and/or other APPs in the use of sound clinical judgment when managing the member’s care.
• Monitor staff performance and complete performance reviews for Care Management RNs.
• Collaborate with, Director of CSNP and Director of ISNP and Clinical Services in carrying out weekly internal and external ICT meetings.
• Collaborate with Director of CSNP and Director of ISNP and Clinical Services on specific development plans for nurses or APP’s on the Care Management team.
Management Responsibilities:
• Determine staffing requirements.
• Recruit, hire and onboard/train new staff.
• Supervise direct reporting staff according to overall company policy.
• Set employee goals and objectives.
• Monitor and conduct performance evaluations of staff at the end of the 90 day introductory period, at established development plan dates and at annual anniversary dates for own direct reports.
• Delegate work duties to staff to attain objectives.
• Provide coaching, disciplinary/corrective action as necessary up to termination.
• Develop staff to maximize potential.
• Develop department procedures.
• Apply strategic planning to determine company, department or unit objectives.
• Collaborate with and contribute to the Leadership Team.
Education: Applicable licenses in the State of New York (NP or PA)
Experience:
· Experience in geriatric nursing or home care with the understanding of problems associated with caring for patients with co-morbid chronic conditions including frailty and experience communicating with patients’ families is preferred.
· Experience in chronic disease management.
· Familiarity with managed care systems and knowledge of personal computer software is desirable.
· Management/supervision experience preferred.

• Effective clinical presentation skills.
• Astute clinical judgment and insight into subtle changes in member status with decisiveness in decision making pertinent to the needs of members and their families.
• Outstanding written and verbal skills, ability to handle politically sensitive issues.
• Requires outstanding people and communication skills. Must be able to build and maintain collaborative, cooperative, and productive relationships with all internal and external customers including members and their families, nursing facility staff, physicians, the health plan, and any other stakeholders in the community.
• Ability to meet and maintain necessary requirements per facility guidelines based on job assignment. These requirements may be outside the scope of Family Choice policies.
• Experience in the development and execution of processes to improve staff workflow, efficiency and effectiveness.
• Ability to effectively delegate and maintain oversight while also being an active contributor to final work product.

Pay: $115,000.00 - $125,000.00 per year
