Salary Range: $20.50 - $21.00 per hour

This position is responsible for providing administrative support to the care management programs at the direction of the management team.
Duties below are typical for this role; actual tasks will depend on the member and the program under which FCNY is providing care management services to that member.

EDUCATION: Associate’s or Bachelor’s Degree in Business Administration Preferred.

EXPERIENCE/ ABILITIES:
• Minimum five (5) years working in a secretarial/administrative support role in a health care setting.
• Computer experience with creating word processing documents, spreadsheets, and presentations.
• Able to problem solve and work with minimal supervision.
• Able to perform basic accurate mathematical skills.
• Able to provide excellent customer service when answering phones and assisting callers.
• Requires outstanding written and verbal skills.
• Requires outstanding people and communication skills.
• Must be able to build and maintain collaborative, cooperative, and productive relationships with all internal and external customers including members and their families, nursing facility staff, physicians, the health plan and any other stake holders in the community.

TASKS:
• Provide administrative support to staff for care management programs.
• Coordinate and attend CSNP IDT – prepare list, coordinate tracker, and document deliverables and team recommendations for weekly IDT.
• Notifies team of transitions of care that occur (both admissions and discharge).
• Assist in the preparation and completion of CSNP agenda and meeting minutes, materials, document meeting minutes, and ensure appropriate distribution to the team.
• Ensure all new enrollees are reviewed during IDT after initial HRA is completed and assure that an ICT note is entered by the CC or RN after the IDT meeting.
• Strong administrative support to care management team; assists with obtaining information and distribution of requested materials such as spreadsheets and/or member lists.
• Complete level and case owner changes as directed by management.
• Schedule required CSNP meetings utilizing ZOOM/Teams as requested, assure that appropriate documents are available for sharing during meeting, and maintain record of meeting attendance.
• May assist with distribution of documents and routine correspondence when requested as back up.
• Provide cross-coverage for other administrative staff within the organization i.e. Administrative Assistant, etc.
• Other duties as assigned.
