LPN (Licensed Practical Nurse) Care Coordinator:Manage hospital discharge and schedule appointments, Gaps and Care Management. $26-$28/hour
These positions are full time Monday-Friday, no evenings or major holidays. Valid license as a Licensed Practical Nurse is required for Nursing positions; experience in Primary Care required; Medent EMR.
