Salary Range: $48,750/yr

The Counselor provide spiritual counseling and education services to patients and families admitted to the hospice program

Duties & Responsibilities

• Demonstrate the ability to provide the following services:
1.Therapeutic interventions with individuals, families and groups for spiritual care and crisis interventions
2. Accurate and timely assessments, documentation, counseling interventions, plans of care and referrals according to policy and procedure and compliance with all State, Federal and Third
Party payer regulations
3. Perform memorial/funeral services in the absence of clergy upon request of family or Director
4. Continuing education for personal and professional growth
5. Provide educational sessions to staff and volunteers as requested.
• Consult/defer to the Director in response to elevated or complex patient/family dynamics
• Coordinate with Interdisciplinary team in assessments, admissions, services, and makes recommendations with regard to the development of the plan of care. Provide continuity of care to •
Patients and their families
• Participate in hospice special memorial events– i.e. Day of Remembrance, Hospice Garden Memorials, etc.
• Coordinate travel time for visits and manages caseload in a cost-effective manner.
• Attend Interdisciplinary meetings
• Participate in evening/weekend on-call or overtime as required, conducting on-call services in a clinically competent and responsive manner.
• Consult with Director when suspected abuse/neglect is suspected and report to the local county department of health services or the appropriate authorities as per state law.
• Perform other functions, attends meetings and conferences, and shares in the support development and community education of the Hospice concept and philosophy, as requested.
• Maintain a professional presence at work and in community by upholding the purpose, goals and objectives of Niagara Hospice.

Qualifications
• Minimum of a Master’s Degree in Counseling, Thanatology, Social Work, Theology, Mental Health, Psychology, Human Services or related degree from accredited college or University.
• Minimum of three years direct care, counseling, education, or training experience with concentration in bereavement, grief and loss or chaplaincy preferred.
• A minimum of one-year direct care experience with a human-service agency preferred.
• Emotional Intelligence, Sound judgment and problem solving skills
• Personal responsibility and integrity
• Good communication, interpersonal and interdisciplinary skills
• Understanding of cultural and ethnical differences
• Ability to use computer to create written and electronic material
• Ability to effectively use social media
• Ability to analyze and interpret assessment data
• Ability to organize and plan
• Be open to New and innovative approaches to meeting the needs of Pt and family members
• Ability to use copier, fax machine, telephone, computer

Licensure/Certification
• Valid Driver’s License
